29 CFR 1910.133(a)(3): The employer did not ensure that each affected employee, who wore prescription lenses while engaged in operations that involved eye hazards, used protection that incorporated the prescription in its design, or wore eye protection over the prescription glasses or protective lenses with side shields: On or about May 30, 2023, and at times prior thereto, the employer did not ensure employees with prescription eyeglasses used eye protection when working with woodworking machines. This conditions exposed employees to eye damage.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed: On or about May 30, 2023, and at times prior thereto, employee used electrical junction box that had unused openings in the two below listed locations of the woodworking shop: 1. The power outlet attached to the room center support pole behind the sander/planer. 2. On the electrical power switch on the outside wall near the window with the large fan installed.
