29 CFR 1910.28(b)(1)(i):  Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):  a) At the site in the raised tank platform area of the  liquid department, on or about 06/05/23, the employer exposed employees to fall hazards in that, the employees were not protected from falling from the walking working surface 12 feet in height. b)At the site in the unload area, on or about 06/05/23, the employer exposed employees to fall hazards in that employees were not protected from falling from the walking working surface, 6-8 feet in height. c) At the site in the trailer washout area, on or about 06/05/23, the employer exposed employees to fall hazards in that employees were not protected from falling from the walking working surface into the wash out pool more than 4 feet below the surface.
