In the Hemi Maintenance Shop of the Swift Creek Chemical Plant, at 15843 S.E. 78th Street, White Springs, Florida:   On or about July 01, 2025, the employer exposed employees to amputation hazards, in that the Kalamazoo Saw did not have a guard properly installed on the unused portion of the blade to provide protection of the point of operation.
