29 CFR  1910.1200(f)(6)(ii): The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical;  (a)  Bay #1, U.S Industrial Lathe, nearest the main office:  On June 8, 2023 and times prior, a plastic squeeze bottle containing lubricating oil, Way Lube, was not marked or labeled with the identity of the contents or the hazard warnings associated with the chemical.  (b)  Bay #1, Kingston Lathe (SN# CH19809):  On June 8, 2023 and times prior, a plastic squeeze bottle containing hydraulic oil was not marked or labeled with the identity of the contents or the hazard warnings associated with the chemical.

29 CFR 1904.41(a)(2): The employer did not electronically submit information from the three recordkeeping forms that you keep under this part (OSHA Form 300A Summary of Work-Related Injuries and Illnesses, OSHA Form 300 Log of Work-Related Injuries and Illnesses, and OSHA Form 301 Injury and Illness Incident Report) to OSHA or OSHA's designee:   (a) McCorkle Machine and Engineering, 125 3rd Ave, Huntington, WV 25701: On or about June 8, 2023, the employer failed, during calendar year 2022, to electronically submit information from their OSHA Form 300A or equivalent by March 3, 2023.
