OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by, crushing, caught between, and amputation hazards due to the lack of safety devices, procedures, and preventative maintenance inspection program:   A.) Precision Metal Products Company, Inc. on or about 6/12/23 and times prior thereto:  Procedures for the setup of powdered metal presses were not developed and utilized. Employees perform setup and die changes.  Powdered Metal Presses include but are not limited to sizing press 9.     B.) Precision Metal Products Company, Inc. on or about 6/12/23 and times prior thereto: the two hand trips on powdered metal presses did not have rings or similar device around them to prevent tampering.  Powdered Metal Presses include but are not limited to sizing press 9.    C.) Precision Metal Products Company, Inc. on or about 6/12/23 and times prior thereto: regular inspections to include looking for defects and control reliability were not performed and documented on equipment.  Equipment includes but is not limited to sizing press 9.    ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation. A fire extinguisher near the water cooler was missing the zip-tie/tamper seal from around the pull-pin on the extinguisher.
