OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:  a.)	 Precision Metal Products Company, Inc., Front loading area, on or about June 12th, 2023 and times prior thereto:  An employee was potentially exposed to struck-by or crushing hazards from a powered industrial truck that was not equipped with a proper operator restraint system.  A Caterpillar/Towmotor brand forklift (Model T40B; Serial# 2N1842) was not equipped with an operator restraint system (i.e.-seatbelt).  The employee operated the forklift for activities that included, but were not limited to, loading, and unloading skids of parts as needed.  ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation.  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records.

29 CFR  1910.22(a)(3): The employer did not ensure that walking-working surfaces are maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice:   a. Precision Metal Products Company, Inc., Various production areas, on or about June 12th, 2023 and times prior thereto, during and after periods of rain, water leaks from the ceiling onto the floor creating potential slip, trip and fall hazards for employees in the area.  These areas included but were not limited to the area next to the sintering oven.  ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR  1910.145(c)(2)(i): Caution signs were not used to warn against potential hazards or to caution against unsafe practices:     a) Precision Metal Products Company, Inc., sintering oven area, on or about June 12th, 2023 and times prior thereto: Employees periodically walked by the side of the sintering furnace where open flames were visible through the openings around the fuel line ports. Caution sings were not posted to alert employees to this condition.   b) Precision Metal Products Company, Inc., various production areas, on or about June 12th, 2023 and times prior thereto:  Employees walked through areas with standing water during and after rain events due to a leaking roof.  Caution sings were not posted to alert employees to potential slip, trip and fall hazards associated with this condition.    ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR  1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet, section 5.8.4:   a)	Precision Metal Products Company, Inc., Front loading area, on or about June 12th, 2023 and times prior thereto:  One compressed gas cylinder of liquified propane was free standing and not secured by any device to prevent the cylinder from falling, rolling, or getting knocked over next to the front loading area bay door.   ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). 29 CFR  1910.178(q)(1): Any power-operated industrial truck not in safe operating condition was not taken out of service, and/or repairs were not made by authorized personnel:    a)	Precision Metal Products Company, Inc., Front loading area, on or about June 12th, 2023 and times prior thereto:  An employee operated a Caterpillar/Towmotor brand powered industrial truck ((Model T40B; Serial# 2N1842) to load skids of parts in the loading area.  The Caterpillar/Towmotor powered industrial truck was leaking hydraulic fluid from the mast area and was in need of mechanical repair.   b)	Precision Metal Products Company, Inc., Front loading area, on or about June 12th, 2023 and times prior thereto:  An employee operated a Caterpillar/Towmotor brand powered industrial truck ((Model T40B; Serial# 2N1842) to load skids of parts in the loading area.  The foot pedals  for the powered industrial truck were worn down to bare metal and did not contain any type of traction material to prevent  the operators foot from slipping off during use.    ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
