Central Ohio Farmers Co-op, Inc., on or about June 7, 2023 at the address of  500 W. Marion Rd., Mount Gilead, OH 43338: The employer failed to ensure an entry permit evaluating the workspace for potential hazards, was completed before employees entered a bin. Employees performing cleaning task inside the bin in this condition were exposed to potentially hazardous energy sources and potentially oxygen deficient atmosphere.

Central Ohio Farmers Co-op, Inc., on or about June 7, 2023 at the address of  500 W. Marion Rd., Mount Gilead, OH 43338:  The employer failed ensure the sweep auger was deenergized and locked out prior to employees entering the bin. Employees performing cleaning task inside the bin in this condition were exposed to crushing/caught in hazards.
