29 CFR  1910.178(l)(2)(ii):The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace:  Central Ohio Farmers Co-op, Inc. located at 500 West Marion Road in Mount Gilead, Ohio;  On or about June 15, 2023, the employer did not ensure forklift training consisted of a combination of formal instruction, practical training, and an evaluation of the operator's performance. Employees were exposed to struck by and crushing hazards in that employees operating forklifts were not provided sufficient training on the use of forklifts.

29 CFR  1910.178(l)(4)(iii):An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years:  Central Ohio Farmers Co-op, Inc. located at 500 West Marion Road in Mount Gilead, Ohio;  On or about June 15, 2023, the employer did not conduct an evaluation of each forklift operator's performance at least once every three years. Employees were exposed to struck by and crushing hazards in that forklift operators did not received an performance evaluation in safe operations and recertification every three years as required by paragraph (l) of this section.
