29 CFR  1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section:  On or about June 12, 2023, at the establishment located at 202 South Johnson Street, Berlin, Wisconsin; the employer did not ensure that an energy control procedure for the 3010 Press (SN H310050205843175) was developed, documented and utilized by an employee performing servicing and/or maintenance operations, such as but not limited to cleaning the screen press bed, to control potentially hazardous energy, such as but not limited to electrical, gravity, and pneumatic, exposing an employee to hazardous energies.

29 CFR  1910.147(d)(3): All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s).  a) On or about June 12, 2023, at the establishment located at 202 South Johnson Street, Berlin, Wisconsin; the employer did not ensure that an employee performing servicing and/or maintenance tasks, such as but not limited to cleaning the screen press bed on the 3010 Press (SN H310050205843175), had isolated all energy sources, such as but not limited to electrical, gravity, and pneumatic energies, exposing an employee to hazardous energies.

29 CFR  1910.212(a)(1): One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  On or about June 12, 2023, at the establishment located at 202 South Johnson Street, Berlin, Wisconsin; the employer did not provide one or more methods of machine guarding to protect the inspector and other employees in the machine area in the following instances:  a) At the outfeed, the Viper Press was not guarded at the ingoing nip points on the belt, exposing employees to caught-in and entanglement hazards. b) At the outfeed, the 520 Press was not guarded at the ingoing nip points on the belt, exposing employees to caught-in and entanglement hazards.
