29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On or about May 25, 2023, and times prior thereto, employees taken measurements for truss installation were not provided fall protection and exposed to an unprotected fall hazard of approximately 9 feet to the lower level.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:(a).  On or about May 25, 2023, and times prior thereto, a portable ladder used to access the upper working surface did not extend at least three feet above the upper landing, exposing employees to fall hazards.

29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards.  On or about May 25, 2023, and at times prior thereto, employee(s) were not trained to recognize fall hazards and steps to be taken in order to minimize observed or known hazards. The employees were exposed to fall hazards of at least nine feet to the lower level while performing required job duties related to construction.  Alvaro Munoz was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.503(a)(1), which was contained in OSHA inspection number 1505754, citation number two (2), item number one (1) and was affirmed as a final order on December 30, 2020, with respect to a workplace located at 2500 River Park Drive, Fort Worth, TX.
