29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access.  Employees were exposed to fall hazards.  Employee(s) engaged in residential framing activities at 20268 Grandview Str, Bucyrus, Kansas were exposed to falls from approximately 13 feet from the roof to the ground below.  Werner aluminum ladders provided to access the roof did not extend at least 3 feet above the eve of the roof.

Bucyrus, Kansas were exposed to fall hazards.  The JLG all-terrain forklift was used to lift employee(s) more than 13 feet to perform framing activities while standing on a pallet.  No fall protection was utilized.

Stair rails were not provided in the following locations:  a) In the basement of the structure, 6 riser stairway was not provided stair rails. b) Between the first and second levels, 8 riser stairway was not provided stair rails. c) Between the second and third levels, 8 riser stairway was not provided stair rails.
