On or about June 22, 2023, employees installed concrete forms in an excavation four (4) feet or more in-depth and more than 25 feet long without a safe means of egress.

On or about June 22, 2023, employees on the south side of the site were concrete forming tasks around uncapped rebar, creating impalement hazards for the workers.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface, and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  On or about June 22, 2023, employees entered and exited the work area on the site's south side using a job-built ladder not extended three feet above the landing.
