On or about June 22, 2023, at the jobsite located at 6084 Alligator Lake Shore West, Saint Cloud, Florida, employees involved in the constructing of a retention wall were exposed to crush-by hazards, in that, the employer failed to instruct employees in the recognition and avoidance of unsafe conditions and the regulations applicable to working in excavations and the impact to adjacent structures.

On or about June 22, 2023, at the jobsite located at 6084 Alligator Lake Shore West, Saint Cloud, Florida, employees were exposed to a crush-by hazard while constructing a retention wall, in that, the employer failed to stabilize a concrete column when the soil and adjacent support system were removed during excavation operations.

On or about June 22, 2023, at the jobsite located at 6084 Alligator Lake Shore West, Saint Cloud, Florida, employees were exposed to a crush-by hazard while constructing a retention wall, in that, the employer failed to have a competent person inspect the adjacent areas such as a concrete column when the soil and adjacent support system were removed.
