29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees: a. Fabritech Solutions, LLC - Oberlin, Ohio: On or about June 27, 2023, the employer did not ensure that employees were protected from electrical hazards in that current-carrying parts of a 3-phase 50A Hubbell plug end device, used to power the Roll-In Saw (model DT1340; s/n 08004DT) bandsaw, were not safeguarded. An employee sustained fatal electrical injuries after contacting live parts of the 480-volt plug connection. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

Fabritech Solutions, LLC - Oberlin, Ohio: On or about April 3, 2023, the employer did not ensure that a 3-phase 50A Hubbell plug end device was installed and used according to listing instructions in that the plug device was not used with a hard service round type cord. The plug was installed and used with unsupported Metal-Clad Cable (Type MC) for power to the 480V Roll-In Saw (model DT1340; s/n 08004DT) bandsaw, exposing employees to electrical shock hazards. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment: a. Fabritech Solutions, LLC - Oberlin, Ohio: On or about June 27, 2023, the working space and access to the 480-volt electrical equipment located on the back side of the Roll-In Saw (model DT1340; s/n 08004DT) bandsaw was not maintained. The east wall and tools, such as a ladder and grinder, stored within the space hindered an employee's ability to access or perform work on the machine's electrical equipment. b. Fabritech Solutions, LLC - Oberlin, Ohio: On or about June 27, 2023, the employer did not ensure that access to the main electrical panelboard, located on the east wall, and controls power sources in the welding/fabrication area, was maintained. Access to the electrical panelboard was obstructed by welding equipment, hindering an employee's ability to access the equipment in the event of an emergency or perform work on the panels as needed. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. Fabritech Solutions, LLC - Oberlin, Ohio: On or about June 27, 2023, the employer did not ensure that employees who are exposed to electrical hazards when installing electrical wiring and components were trained in electrical safety-related work practices as required by 1910.331-1910.335. Employees performing work on or near circuits of the 480V Roll-In Saw (model DT1340; s/n 08004DT) bandsaw which were, or may have been energized, were not trained in the skills necessary to distinguish exposed live parts from other parts of electrical equipment. b. Fabritech Solutions, LLC - Oberlin, Ohio: On or about June 20, 2023, the employer did not ensure that employees who are exposed to electrical hazards when performing electrical tests were trained in electrical safety-related work practices as required by 1910.331-1910.335. Employees performing work on or near circuits of the 480V shop-made hydraulic press which were, or may have been energized, were not trained on electrical shock, arc flash hazards, and protective equipment to protect against electrical hazards. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
