29 CFR  1910.178(q)(1): Any power-operated industrial truck not in safe operating condition was not taken out of service, and/or repairs were not made by authorized personnel:  a) Warehouse:  On or about June 27, 2023, and at times prior,  employees were exposed to electric shock hazards, in that the employer did not remove a powered pallet jack, model 1-24020-04-HF (serial number 2311600002), from service until repaired where the pallet jack was was missing a cover for electrical components and was used by employees to move merchandise.

29 CFR  1910.178(q)(7): Industrial trucks were not examined before being placed in service:  a) Warehouse:  On or about June 27, 2023, and at times prior, employees were exposed to electric shock hazards, in that the employer did not require and ensure that employees conduct examinations of powered pallet jacks before being placed in service including, but not limited to, powered pallet jack, model 1-24020-04-HF (serial number 2311600002) where the pallet jacks are used by employees to move merchandise.
