29 CFR 1926.416(a)(1): Employee(s) were permitted to work in proximity to an electric power circuit that the employee could contact in the course of work and was not protected against electric shock by de-energizing the circuit and grounding it or by guarding it effectively by insulation or other means:  a) On or about June 27, 2023, at the road intersection of Sage Glen Ln and W. Pinhook located at 2420 W Pinhook Rd, Lafayette, LA 70508; an employee installed an exterior light fixture to the streetlight of the intersection in close proximity to live power lines that were not de-energized.

29 CFR 1926.416(a)(3): The employer began work without determining if employees could contact any part of an energized circuit and did not post warning signs and advise employees of the location of the circuit, hazards, and protective measures they need to take.   a) On or about June 27, 2023, at the road intersection of Sage Glen Ln and W. Pinhook located at 2420 W Pinhook Rd, Lafayette, LA 70508; the employer failed to determine if employees engaged in street lighting upgrades could come in contact with unprotected energized powerlines associated with electrical hazards.
