1910.272(e)(1)(ii)  Current employees, and new employees prior to starting work in the grain handling facility were not trained in the specific procedures and safety practices applicable to their job tasks including but not limited to cleaning procedures.     On or about July 4, 2023, and at times prior thereto at  106 FM 796 TYNAN, TX , employees used the bins to store and transfer corn and other grains during loading and unloading transfer trailer trucks. Employees were exposed to crushing hazards from a collapse of a storage bin #3 due to compromised leaking bins that affect the stability of the bin system.

29 CFR  1910.272(g)(1)(ii): All mechanical, electrical, hydraulic, and pneumatic equipment which presents a danger to employees inside grain storage structures shall be deenergized and shall be disconnected, locked-out and tagged, blocked-off, or otherwise prevented from operating by other equally effective means or methods.   On or about July 4, 2023, and at times prior thereto at 106 FM 796 TYNAN, TX employees conducted grain handling facility repairs at this workplace  such as but not limited to  removing and replacing motor pulley belts did not implement procedures for the use of tags and locks which would prevent the inadvertent application of energy or motion to the equipment being serviced or repaired.
