29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b): a. On or about July 13, 2023, at the above addressed jobsite, employees performing residential roofing operations, approximately eleven (11) feet above the ground, were not protected from falls, thereby exposing the employee to fall hazards. b. On or about July 13, 2023, at the above addressed jobsite, an employee performing residential roofing operations, approximately twenty (20) feet four (4) inches above the ground, was not protected from falls, thereby exposing the employee to fall hazards.

29 CFR 1926.502(d)(15): Anchorage points used for attachment of personal fall arrest equipment were not independent of any anchorage point being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee: a. On or about July 13, 2023, at the above addressed jobsite, employees conducting roofing operations on a roof were improperly tied off in that, the anchorage point for the personal fall arrest system (PFAS) was not capable of supporting at least 5000 pounds. The employees installed two (2) ropes to one single anchor point on the roof, thereby exposing the employees to fall hazards of approximately eleven(11) feet.
