This was last demonstrated on or about July 11, 2023, when employees were permitted to work from a fabricated tubular scaffold that was not fully planked or decked. Employees were tuckpointing on a commercial building and were only standing on one or two wooden planks. The employer failed to protect employees by not ensuring that the employees had a working platform on all working levels fully planked or decked between the front uprights and the guardrail supports.

This was last demonstrated on or about July 11, 2023, when employees were permitted to work from a tubular scaffold that was not inspected for defects. Employees were tuckpointing on a commercial building and were working from the scaffold that was missing connectors on the poles, legs, posts, frames, and uprights; bracing, planking and decking. The employer failed to protect employees by not ensuring that a competent person was inspecting the scaffold before each shift.

This was last demonstrated on or about July 11, 2023, when employees were permitted to work from a tubular scaffold that was closer than the minimum distance to live overhead insulated wiring of 120 - 240 volts. Employees were tuckpointing on a commercial building and were working from the scaffold that was within one to two feet from an overhead power line. The employer failed to protect employees by not ensuring that incidental contact with the live power lines could be made with the scaffold or employees with a minimum distance of three feet.
