Flammable liquids, including stock for sale, were stored in a location that limited the use of exits, stairways, or areas normally used for the safe egress of people:  (a) On or before 7/21/2023, the employer did not keep all exits free and clear of fire hazards in that four, five-gallon plastic gasoline cans and one two-gallon gasoline can were stored next to the dock bay doors where trucks were loaded and unloaded by forklifts. This condition exposed employees to limited egress and fire hazards.

29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered:  (a) On or before 7/21/2023, the employer did not ensure that all forklift attachments were approved by the forklift manufacturers and the changes in load center reflected on data plates. Forklifts were used with unapproved fork extensions as well as bale spears. This condition exposed employees to impalement, crushing, and struck-by hazards.
