A) 1 Crossman Rd North Sayreville, NJ 08870 - The employer did not ensure locks were affixed to isolate all energy sources on the spindle carrier at stand #14. This condition exposed employees to an amputation hazards. Incident occurred on July 19, 2023.

A) 1 Crossman Rd North Sayreville, NJ 08870 - The procedures developed to lock out Stand # 14 and the spindle carrier did not clearly outline the steps to shut down and isolate all energy sources to protect employees from unexpected energization or start up of the equipment . This condition exposed employees to amputation hazards. Incident occurred on July 19, 2023.
