29 CFR  1910.147(c)(4)(i): Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in the activities covered by this section.  On or about July 24, 2023, employees of a maintenance shop were exposed to a struck-by and crush -by hazards at a worksite located at 1900 E Blaine St, Springfield MO.  Employees engaged in maintenance and repair work of a suspension system of a bus did not follow the company's lockout / tag-out step-by-step procedures. Employees were exposed to struck-by and crush-by hazards.

29 CFR  1910.147(d)(4): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  On or about July 24, 2023, employees of a maintenance shop were exposed to a struck-by and crush -by hazards at a worksite located at 1900 E Blaine St, Springfield MO. Employees were allowed to work in and around a bus that was being repaired without isolating and locking out all sources of energy including but not limited to pneumatic, mechanical and electrical energy.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation.  On or about July 24, 2023, employees of a maintenance shop were exposed to a struck-by and crush -by hazards at a worksite located at 1900 E Blaine St, Springfield MO. A maintenance technician was allowed to independently conduct maintenance and repair tasks without having completed authorized employee training in recognition, isolation, and control of residual and hazardous energy and the procedures for locking out all sources of energy including but not limited to pneumatic, mechanical and electrical energy.
