29 CFR 1910.269(m)(3)(ii): The employer did not ensure that all switches, disconnectors, jumpers, taps, and other means through which known sources of electric energy might be supplied to the particular lines and equipment were de-energized / inoperable (i.e. open) and then did not ensure that such means were tagged to indicate that employees were at work:  a) On or about July 13, 2023, and at times prior, ENSCO, Inc., as the controlling employer, did not, at any point before or during work operations in Substation 2, ensure the Alternating Current switchgear main circuit breaker was in the open position, thereby eliminating power transfer backfeeds to points upstream of the breaker (ie., Medium Voltage Side of primary transformer T201) from downstream sources (ie., transformer T203, pneumatic oil circuit breaker system and DC voltage from the Transit Test Track).  This exposed employees of LND Technical Services, LLC, to electrical hazards while performing work on Substation 2 equipment.
