Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees employment and a place of employment which was free from recognized hazards of being struck-by which were causing or likely to cause death or serious physical harm to employees: a) This violation occurred on or about August 1, 2023, at a worksite located at 3452 Flair Drive, Dallas, TX, where an employee who was working and walking within the turn radius of the Watson 1100 CM2, SN 22332 drill rig equipment, was exposed to struck-by hazard. b) This violation occurred on or about August 1, 2023, and at times prior thereto; when employees operating Watson 1100 CM2, SN 22332 drill rig equipment, using the hook in the manner other than that for which it is intended, exposing employees to struck-by hazard.
