29 CFR 1910.27(b)(1)(ii): The employer did not ensure that no employee uses any anchorage before the employer has obtained written information from the building owner that each anchorage meets the requirements of paragraph (b)(1)(i) of this section. The employer did not keep the information for the duration of the job.  Fire Station 1 Hose Tower: A scaffold vertical post was used to anchor a rope decent system for firefighter rescue training. Employees were exposed to fall hazards.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  Fire Station 1 Alarm Room: Two radio charging stations that consisted of six charging ports each at three amps per port (thirty-six amps total) were plugged into a relocatable power tap designed for low amperage equipment. Employees were exposed to fire and electrical hazards.

Station 1 Galley: The electrical junction box on the southeast wall did not have a cover. Employees were exposed to electrical hazards.  b. Station 2 Day room: The electrical junction box under the training computer did not have a cover. Employees were exposed to electrical hazards.
