29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  On or about August 8, 2023, employees were exposed to machine hazards in the following instances:  a.) An employee was exposed to caught between hazards when performing tool set up and maintenance activities on the Peddinghaus 863-D Anglemaster Automated Punch Press. The employer failed to implement energy control application steps as the machine was not isolated from electrical, hydraulic, and pneumatic energy sources prior to performing the setup and maintenance activities [per the 1910.147(d)(3) requirements]. As a result, the remaining applicable energy control elements, involving LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine hazards.  b.) An employee was exposed to machine hazards associated with moving parts when performing maintenance and housekeeping activities on or near the Beam Line's Peddinghaus PCD 1100/3C ATC Automated Drilling Machine and the connected Beam Transfer roller line. The employer failed to implement energy control application steps as the machines were not isolated from electrical and hydraulic energy sources prior to performing the maintenance activities [per the 1910.147(d)(3) requirements]. As a result, the remaining applicable energy control elements, involving LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine hazards.
