29 CFR 1910.22(a)(2): The employer did not ensure that the floor of each workroom is maintained in a clean and, to the extent feasible, in a dry condition.  The employer did not ensure that when wet processes are used, drainage is to be maintained and, to the extent feasible, dry standing places, such as false floors, platforms, and mats are provided:   a) Hollywood Meadowland Casino Eatery Food Court Mercato, on or about August 29, 2023 - The tile floor under the pizza preparation area was slippery from flour dust and debris.   b) Hollywood Meadowland Casino Barstool kitchen areas, on or about August 29, 2023 - The kitchen floor was covered with food, debris, grime, and water presenting an ongoing slippery floor surface.    c) Hollywood Meadowland Casino H Lounge Keg Beer Storage Cooler, on or about August 29, 2023 - The floor surface was wet which created slippery floor surface.   d) Hollywood Meadowland Casino H Lounge Service Bar, on or about August 29, 2023 - The floor surface had standing water under and near the ice holder and was not immediately cleaned up which created a slippery floor surface.  e) Hollywood Meadowland Casino 2Ten Bar Keg Beer Storage Area, on or about August 29, 2023 - The ramp floor surface leading to the storage room was covered with water and debris which created a slippery floor area.   f) Hollywood Meadowland Casino Barstool Kitchen/Rear end cooler, on or about August 29, 2023 - Free standing water was directly under the sink creating a slippery floor surface.

29 CFR  1904.41(a)(2):  The employer had not electronically submitted OSHA Form 300A Summary of Work-Related Injuries and Illnesses when establishment with 250 or more employees at any time during the previous year and in designated industries; to OSHA designee when were required under this part once an year and no later than the date March 2, 2023:  a)  In the Hollywood Casino at the Meadows located at 210 Racetrack Rd., Washington  PA, on or about August 9, 2023 - the employer failed during calendar year 2023, to electronically submit information from their  2022 OSHA Form 300A or equivalent by March 2, 2023.  The establishment employed 623 employees and is classified 713210 NAICS during calendar year 2022.
