29 CFR  1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:   a) Excavation located on Massee Ln.: On or about August 10, 2023, the employer exposed employees to engulfment, struck-by and cave-in hazards, in that; it did not instruct employees to recognize and avoid unsafe conditions, including, but not limited to, improper construction of trench shields, failure to utilize the necessary personal protective equipment, and a lack of training and recognition in safe egress procedures while performing work  at trench excavation site.
