29 CFR  1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied:  a. 51-61 Verona Ave., Newark, NJ   The employer did not ensure that a working platform attached to a JLG, Model G12-55A Telehandler was secured to the forks. Employees engaged in construction activities, where materials were being unloaded, worked from the platform that was not fully secured to the forks and carriage.  Violation occurred 08/14/2023

29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a. 51-61 Verona Ave., NEWARK, NJ   The employer failed to ensure that the employees were protected from falling from an unprotected edge while pouring concrete into a salt dome forms under construction. Employees worked at a height of approximately 20 feet from the ground without utilizing any means of fall protection.  Violation observed 08/14/2023
