On or about August 16, 2023 at the address of 4420 Alliance Drive, Ontario, OH 44906: The employer failed to instruct employee(s) in the recognition and avoidance of hazards commonly found in their scope of work. Employee were not trained to recognize and avoid hazards within their work environment such as, but not limited to: fall from heights and struck-by hazards while performing steel erection work activities.

Employees utilizing the grinder in this condition were exposed struck-by hazards.

On or about August 16, 2023 at the address of 4420 Alliance Drive, Ontario, OH 44906: The employer did not ensure that aerial lift operator donned and secured their personal fall arrest systems before operating JLG All-Terrain aerial lift (model # 400S, serial # 0300305430). Employee utilizing aerial lift to performed steel erection work activities (installing sidewall girt) was exposed to fall hazards of between 22ft - 25ft or greater to the ground below.

29 CFR  1926.453(b)(2)(iv): Employee(s) working in an aerial lift were not standing firmly on the floor of the basket:  a)	Lozano Welding and Fabrication LLC. On or about August 16, 2023, at the address of 4420 Alliance Drive, Ontario, OH 44906: The employer failed to ensure that employees worked from the platform of an aerial lift. Two employees utilizing aerial lift (model # 400S, serial # 0300305430) to perform steel erection activities (installing sidewall girt) utilized the basket's mid/top rails as a means of increasing their working height. Employees were exposed to fall hazards of between 22ft - 25ft or greater to the ground below.   Lozano Welding and Fabrication LLC was previously cited for a violation of this Occupational Safety and Health Standard 1926.453(b)(2)(iv), which was contained in OSHA inspection number 1672435, citation number 1, item number 1 and was affirmed as a final order on July 13, 2023 with respect to a workplace located near 1118 Central Park Drive, O'FALLON, IL 62269.
