29 CFR  1910.28(b)(3)(i): The employer did not ensure that each employee is protected from falling through any hole (including skylights) that is 4 feet (1.2 m) or more above a lower level by one or more of the following methods listed in 29 CFR 1910.28(b)(3)(i)(A)-(D):   On or about August 19, 2023, employees were exposed to a fall hazard of more than 4 feet above the ground, without a means of fall protection while prepping to pressure wash the employer's roof.

29 CFR  1910.28(b)(13)(i): The employer did not ensure that when work is performed less than 6 feet (1.6 m) from the roof edge, each employee is protected from falling by a guardrail system, safety net system, travel restraint system, or personal fall arrest system.   On or about August 19, 2023, employees were exposed to a fall hazard while carrying pressure washing equipment less than 6 feet from a roof's edge without a means of fall protection.
