29 CFR 1910.304(g)(5): Grounding path. The path to ground from circuits, equipment, and enclosures shall be permanent, continuous, and effective.  Nabor's Rig X04 Drill site: On or about August 22, 2023, grounding rods in multiple instances were not effectively installed into the ground. This exposed employees to electrical shock hazards.  a)  The grounding rod on the Directional trailer was approximately 3-1/2 feet into the ground.  b)  The grounding rod on the Drilling Supervisor's trailer was approximately 3-feet 5-inches into the ground.  c)  The grounding rod in between Night Company Man's trailer and Mud sampling trailer was approximately less than 1-1/2 feet into the ground.  d)  The grounding rod on the MDW Trailer was approximately 4 feet into the ground.

29 CFR 1910.305(g)(2)(iii): Cables were not connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws.   Nabor's Rig X04 Drill site: On or about August 22, 2023, employees were exposed to electrical shock hazards when the employer did not ensure the strain relief provided prevented pulling of the electrical wires in the following instances.  a) The air conditioner's 220/240V electrical box on Night Company Man's Trailer had a damaged strain relief.  b) The 220/240V electrical box at the Directional Trailer had a damaged strain relief.

29 CFR 1910.29(f)(4): Openings in stair rails. The employer did not ensure that no opening in a stair rail system exceeds 19 inches (48 cm) at its least dimension.   Nabor's Rig X04 Drill site: On or about August 22, 2023, employees were exposed to fall hazards when the employer failed to ensure that there was no opening in a stair rail system exceeded 19 inches in the following instances:  a) Mud engineers' trailer  b) MWD trailer  c) Trailer adjacent to the company's trailer  Hoppes Construction LLC was previously cited for a violation of this occupational safety and health standard 1910.29(f)(4) which was contained in OSHA inspection number 1661853, citation number 1, item number 1 and was affirmed as a final order on June 12, 2023, with respect to a workplace located at Unit 405, 20.6 miles NW direction from Pecos, Pecos, Texas 79772.

29 CFR 1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy:  Nabor's Rig X04 Drill site: On or about August 22, 2023, the employer did not ensure each outlet had a suitable faceplate. This exposed employees to shock hazards.  a) Directional Drilling Trailer, the outlet cover did not have the correct faceplate exposing live electrical parts.  b) Mud Sampler's Trailer backside, the outlet cover did not have the correct faceplate exposing live electrical parts. The outlet cover would not close due to an obstruction.
