29 CFR 1910.269(c)(2):  The job briefing did not cover at least the following subjects:  hazards associated with the job, work procedures involved, special precautions, energy-source controls, and personal protective equipment requirements:  a) On or about September 1, 2023 at the Lagoon Creek Combustion Turbine employees performing fuel gas supply energy isolation were exposed to fire and struck-by hazards and the employer did not ensure the job briefing covered all the hazards associated with the job, and work procedures involved.

29 CFR 1910.269(d)(6)(ii):  The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment.  An orderly shutdown was not used to avoid any additional or increased hazards to employees as a result of the equipment stoppage:  a) On or about September 1, 2023 at the Lagoon Creek Combustion Turbine employees performing fuel gas supply energy isolation utilized procedures that did not avoid any additional or increased hazards to employees as a result of the equipment stoppage in that ignition sources were not eliminated or controlled and struck-by hazards were created through the venting of high pressure natural gas without decreasing the pressure as much as possible prior to opening of the vent pipes.
