29 CFR 1910.268(b)(7): Approach distances to exposed energized overhead power lines and parts. The employer shall ensure that no employee approaches or takes any conductive object closer to any electrically energized overhead power lines and parts than prescribed in Table R-2, unless: a. Located along the powerline easement in the vicinity of 11250 Township Road 67, Killbuck, Ohio: On or about September 06, 2023, employees were throwing a conductive metal roller attached to bull-line pull woven locatable tape over the fiber optic support strand located approximately 5 feet below an energized 7,200-volt powerline. The employer did not ensure that employees did not take a conductive object within the minimum approach distance, thereby exposing employees to electrical shock hazards.

29 CFR 1910.268(e): Tools and personal protective equipment -- Generally. Personal protective equipment, protective devices and special tools needed for the work of employees shall be provided and the employer shall ensure that they are used by employees. Before each day's use the employer shall ensure that these personal protective devices, tools, and equipment are carefully inspected by a competent person to ascertain that they are in good condition. a. Located along the powerline easement in the vicinity of 11250 Township Road 67, Killbuck, Ohio: On or about September 06, 2023, the employer did not ensure that employees used an insulating fiberglass layup stick, known as the FG-Series Extension Pole, to attach a fiber optic cable to a support strand to ensure that employees were insulated from an energized 7,200-volt overhead powerline, thereby exposing employees to electrical shock hazards.
