29 CFR  1910.147(d)(4): Lockout or tagout device application.  a) On and before December 8, 2023, with regard to an establishment located at 1521 Brookfield Ave, Howard, WI 54313, the employer did not ensure that employees conducting sanitation activities applied lockout or tag out devices on the pneumatic airline while performing hazardous energy control procedures for the Line #50 Weber Texter.   b)  On or about December 8, 2023, with regard to an establishment located at 1521 Brookfield Ave, Howard, WI 54313, the employer did not ensure that employees conducting sanitation activities applied lockout or tag out devices on the Nitrogen and Pneumatic lines for the Line #30 Bagger.

29 CFR  1910.147(f)(4): Shift or personnel changes. Specific procedures shall be utilized during shift or personnel changes to ensure the continuity of lockout or tagout protection, including provision for the orderly transfer of lockout or tagout device protection between off-going and oncoming employees, to minimize exposure to hazards from the unexpected energization or start-up of the machine or equipment, or the release of stored energy.   On and before December 8, 2023, with respect to an establishment located at 1521 Brookfield Ave, Howard, WI 54313, the employer did not ensure that employees utilized a specific procedure to ensure continuity of lockout or tagout protection during the change from 2nd Shift to 3rd Shift sanitation activities.

29 CFR  1910.212(a)(4):Barrels, containers, and drums. Revolving drums, barrels, and containers shall be guarded by an enclosure which is interlocked with the drive mechanism, so that the barrel, drum, or container cannot revolve unless the guard enclosure is in place.  On and before September 20, 2023, with respect to an establishment located at 1521 Brookfield Ave, Howard, WI 54313, employees were exposed to potential entanglement hazards when the Tumble Drum on Line #20 in the Shred Department was not guarded to prevent contact with the rotating drum or access to pinch points created by the operation of the drum.

29 CFR  1910.212(a)(1):Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.   b) On and before September 20, 2023, with respect to an establishment located at 1521 Brookfield Ave, Howard, WI 54313, employees were exposed to potential caught-in hazards when the in-running nip point on the end of packaging Line #10 was not guarded to prevent potential exposure.  c) On and before September 20, 2023, with respect to an establishment located at 1521 Brookfield Ave, Howard, WI 54313, employees were exposed to potential caught-in hazards when the in-running nip point on the end of packaging Line #20 was not guarded to prevent potential exposure.
