OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees  in that employees were exposed to struck-by hazards from  static pressure contained within caped 24 inch water pipeline.  (a) On or about September 13, 2023,  at a job site located at or around 9300 Sheridan Road, Pleasant Prairie, Wisconsin, employees working in a trench, engaged in activities related to a water main expansion project were exposed to struck-by hazards during the removal of the 24-inch Tyler Union Ductile Iron Cap.   The pressure which was contained inside the water main pipe was not released prior to the removal of the cap.

29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:  (a) On or about September 13, 2023, at a job site located at or around 9300 Sheridan Road, Pleasant Prairie, Wisconsin, employees were working in a trench approximately ten (10) feet deep and approximately 45 feet in length without a means of egress installed to require no more than 25 feet of lateral travel.  Employees were working at the South end of the trench at the watermain end, and would need to travel to the North end of the trench 45 feet away to egress the trench.

29 CFR  1926.652(e)(1)(i): Members of support systems were not securely connected together to prevent sliding, falling, kickouts, or other predictable failure:  (a) On or about September 13, 2023, at a job site located at or around 9300 Sheridan Road, Pleasant Prairie, Wisconsin, two spreader bar pins were missing on the top spreader bar of the South end of the North trench box.  All spreader bar pins required in the trench box were not installed or fully inserted as required by tabulated data, to prevent lateral or other hazardous movement.
