(a) On or about September 13, 2023,  at a job site located at or around 9300 Sheridan Road, Pleasant Prairie, Wisconsin, employees working in a trench, engaged in activities related to a water main expansion project were exposed to struck-by hazards during the removal of the 24-inch Tyler Union Ductile Iron Cap.   The pressure which was contained inside the water main pipe was not released prior to the removal of the cap.
