29 CFR 1926.404(b)(1)(iii)(C):Where an assured equipment grounding conductor program was being utilized, the employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. At a residential worksite located at 5004 NE 56th Ter KC MO -- employees engaged in roofing duties were using an orange extension cord to power a Dewalt skilsaw. The orange extension cord was missing a ground pin and had multiple splices throughout. This exposed the employee to electrical hazards. This was documented on or about 09/19/2023.

29 CFR 1926.1053(b)(1):Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. At a residential worksite located at 5004 NE 56th Ter KC MO -- employees were using 2 extension ladders to gain access to the roof. One in front and the other in back. Neither ladder extended 3 rungs above the landing surface. This exposed the employees to fall hazards in not maintaining 3 points of contact when ascending/descending. This was documented on or about 09/19/2023.
