29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a.) TIP Construction Inc. 150 South Lakeside Dr., North East, PA 16428: Residential roof - on or about 10/04/2023 and at times prior thereto, employees were conducting tear off and patch work operations on a residential roof with an estimated 4:12 pitch. Employees were exposed to a fall hazard of at least 16.74 feet. Employees were not using fall protection.

TIP Construction Inc. 150 South Lakeside Dr., North East, PA 16428: Residential roof - on or about 10/04/2023 and at times prior thereto, employees were conducting tear off and patch work operations on a residential roof with an estimated 4:12 pitch. Employees were exposed to a fall hazard of at least 16.74 feet. Employees were not trained on the fall protection standard subpart M, nature of fall hazards, and falling object protection/placement of storage materials and equipment.

29 CFR  1904.39(a)(1):The employer did not report within 8-hours the death of an employee resulting from a work-related incident   a) TIP Construction Inc. 150 South Lakeside Dr., North East, PA 16428 - On 10/04/2023, the employer failed to report a work-related incident resulting in an employee death within 8 hours to OSHA. The incident occurred on 10/04/2023 around 3 pm with the employee announced deceased around 5 pm on 10/04/2023. The incident was not reported to OSHA until 10/05/2023 at 10:18 am.
