29 CFR  1926.453(b)(2)(v): A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift.  a) New McDonalds located at 5260 Matt Hwy., Cumming, GA 30028: On or about October 10, 2021, the employer exposed employee to ejection/fall hazards of approximately 20-feet, 8-inches to the lower ground level, in that a lanyard with full body harness was not used to connected to the anchor point provided in the Genie S-45/XC Aerial Lift basket.

29 CFR  1926.453(b)(2)(iv): Employees shall always stand firmly on the floor of the basket, and shall not sit or climb on the edge of the basket or use planks, ladders, or other devices for a work position.  a) New McDonalds located at 5260 Matt Hwy., Cumming, GA 30028, Roof: On or about October 10, 2023, the employer exposed employees to fall hazards of approximately 20-feet, 8-inches to the lower level, in that an employee  did not work from the floor of the aerial lift basket while installing decorative trim.  The employee was not connected to an anchor point while using the mid rail as a walking/working surface.
