The employer did not ensure that all exit accesses are at least 28 inches wide at all points.  a) On or about October 12, 2023, the aisle from end of racks 3 and 4 to the fire exit in the New Warehouse Area was completely blocked due to boxes and pallets of product placed in the aisleway.  b) On or about October 12, 2023, the aisle from end of rack 5 and the staging area to the fire exit in the New Warehouse Area was completely blocked due to boxes and pallets of product placed in the aisleway.  c) On or about October 17, 2023, the aisle from end of rack 5 and the staging area to the fire exit in the New Warehouse Area was completely blocked due to boxes and pallets of product placed in the aisleway.  d) On or around October 12, 2023, the aisles from the ends of the small pick racks to the fire exit in the Small Pick Rack area were 5 inches wide at the narrowest point due to boxes, trash, and product placed in the aisleways.
