29 CFR  1910.36(g)(2): An exit access was not at least 28 inches (71.1 cm) wide at all points and the width of the exit and exit discharge was not at least equal to the width of the exit access.  On and or before October 11, 2023, multiple aisles and/or exit access points were not at least 28 inches wide at all points. Employees were potentially exposed to a narrow escape route and fire hazard.  In the following locations: a) Information Management Department, Help Desk Office, H109, (57) Dell PC P2222H monitors were stacked and created a narrow exit approximately 26 inches. b) Lab main room, (2) blue Helmer refrigerators were configured where it created a narrow exit approximately 26 inches.  c) Lab storage room D103, metal shelves were configured where it created a narrow exit approximately 18.5 inches on the right side and 22 inches on the left side.

29 CFR  1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  On and or before October 11, 2023, multiple appliances such as refrigerators and space heaters were plugged into relocatable power taps and/or "daisy chained" in which were not used according to listing and labeling instructions. This condition potentially exposed employee(s) to electrical shock and/or fire hazards.  In the following locations: a) G210 Breakroom, a White Westinghouse Fridge was plugged into a white unlabeled relocatable power tap.  b) H109, Information Management Department, Help Desk Office, back right-hand corner cubical, a Lakewood Heater was plugged into a Triplite relocatable power tap.
