29 CFR  1910.151(c):  Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use. Reference: ANSI Z358.1 Emergency Eyewash and Shower Standard 5.4.2 Reference: The eyewash unit shall be in accessible locations that require no more than 10 seconds to reach, located on the same level as the hazard, and the path of travel shall be free of obstructions that may inhibit the immediate use of the equipment.  Second and Third Floor: On and/or before October 11, 2023, employees used corrosive products where the following was observed:   a.  Multi-Service Ward:  The emergency eye wash unit was located outside the ward work center, across the hallway, and through a badge controlled door that led into the Main Operating Room (MOR) work center. The travel distance was outside the 10 second time and had a badge 'swipe' required to open the MOR self-closing doors limiting the travel path to the eye wash unit.   b.  Laboratory:  The emergency eye wash unit located in the front of the laboratory was blocked by multiple cardboard boxes containing supplies.  These conditions potentially exposed the employees to retinal or corneal burns.

The employer had not provided the respirator user(s) with the information contained in Appendix D which potentially exposed employees to silica and Portland Cement inhalation hazards.
