29 CFR  1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary.  (a) On or about October 17, 2023, at the jobsite located at 8989 North Port Washington Road, Bayside WI, employees were conducting cleaning work on the tubular welded scaffold that was not fully planked on the level the employee was working.  Specifically, an employee worked from one 18 inch wide scaffold plank erected between two six (6) foot wide tubular welded scaffold frames, exposing employees to a fall hazard of 17 feet to 18 feet to the ground below.
