29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)  On or about 10/11/2023, at the jobsite located at 13450 Greengate Blvd,  Fort Myers, Florida 33919, employees were exposed to fall hazards of approximately 17 feet 9 inches, in that, fall protection was not used while engaged in roofing activities while on a second story roof.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) On or about 10/11/2023, at the jobsite located at 13450 Greengate Blvd, Fort Myers, Florida 33919, employees were exposed to fall hazards of approximately 17 feet 9 inches, in that, the extension ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access.

29 CFR  1926.1053(b)(5)(i):Non-self-supporting ladder(s) were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support)  a) On or about 10/11/2023, at the jobsite located at 13450 Greengate Blvd,  Fort Myers, Florida 33919, employees were exposed to fall hazards of approximately 17 feet 9 inches, in that, the angle of the extension ladder in use to gain access to the roof was too shallow, 64.9 degrees.
