29 CFR  1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level.   a. On or about February 17, 2026, at the jobsite: employees were exposed to 19 feet, 8-inch fall hazards, in that fall protection lanyards were not used properly while  installing roofing tiles on a 7:12 pitch roof.

29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a. On or about February 17, 2026, at the jobsite: employees were exposed to 19-foot 8-inch fall hazards, in that, fall protection training was not provided to employees working on a 7:12 pitch roof.
