On top of a structure, located at 12021 Diamond Springs Drive in Jacksonville, Florida: On or about October 24, 2023, the employer exposed an employee to eye injury struck-by hazards caused by flying debris, in that eye protection was not utilized by an employee while using a pneumatic nail-gun.

On top of a residential structure, located at 12021 Diamond Springs Drive in Jacksonville, Florida 32225: On or about October 24, 2023, the employer exposed employees to an 8-foot fall hazard, in that fall protection was not utilized by employees performing re-roofing operations.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a. Positioned in the front of a residential structure, located at 12021 Diamond Springs Drive in Jacksonville, Florida: On or about October 24, 2023, the employer exposed employees to fall hazards, in that a portable ladder did not extend at least 3 feet above the upper landing surface (roof eave) while employees climbed and descended the ladder.
