29 CFR  1926.21(b)(2):The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  a.) At the jobsite, located at 8901 Play Through Drive, #34 Davenport, Fl 33896; On or about 9/15/23, the employer exposed employees to amputation hazards in that training was not provided on recognition and avoidance of unsafe conditions while using unguarded  table saws to perform soffit cutting and installation operations.

29 CFR  1926.300(b)(3):One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.  a.) At the jobsite, located at 8901 Play Through Drive, #34 Davenport, Fl 33896; On or about 9/15/23, the employer exposed employees to amputation hazards in that soffit cutting operations were being conducted while using an unguarded table saw in order to fit and size material during soffit installation on a residential home.

29 CFR  1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.  a) On or about July 30, 2023, at the worksite 1660 Bunting Lane Sanibel, FL 33957, the employer failed to report a work-related amputation of an employee to OSHA within twenty-four (24) hours.
