29 CFR  1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service:  Worksite located at 8500 NW Jefferson St., Kansas City, Missouri- Employees exposed to fall hazards failed to inspect their fall protection equipment prior to each use to look for damage, wear and deterioration. The harnesses and lanyards used by the employees could not be properly inspected due to the age of the equipment, condition of the equipment and the amount foreign materials on the equipment.
