29 CFR  1910.215(b)(9): Guard for abrasive wheel machine where the operator stands in front of the machine was not constructed so that the peripheral protecting member could be adjusted to the constantly decreasing diameter of the wheel.   On November 8, 2023, and times prior thereto, inside the maintenance shop, the employer did not ensure adjustable tongue guards were installed prior to employees operating the bench grinder exposing them to struck-by hazards.

29 CFR  1910.215(c)(1)(v): Regardless of flange type used, the wheel shall always be guarded. Blotters shall be used in accordance with paragraph (c)(6) of this section. (For Figure No. O-24, http://www.osha.gov/OshStd_gif/10OF24.gif ) The Type 27 A Wheel is mounted between flat non-relieved flanges of equal bearing surfaces.   On November 8, 2023, and times prior thereto, inside the maintenance shop, the employer did not ensure the guard for the right side flange was installed prior to employees operating the bench grinder exposing them to caught-by hazards.
