29 CFR  1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees.  On or about February 15, 2024, and at times prior thereto, in the kill room, in wet locations, the employer did not ensure the Jarvis Buster V wiring was free of recognized electrical shock hazards in the following instances:   a. The 42v wire connector to the Jarvis Buster V flexible cord wiring was damaged exposing wiring in multiple locations, and b. The 220V, 3 phase strain relief and wire insulation feeding the 3.3 horsepower motor and strain relief was damaged.

29 CFR  1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling.  On or about February 15, 2024, and at times prior thereto, the employer allowed the use of an electrical power switch installed on a Jarvis Buster V that was not listed or labeled for use for this machine and in a wet environment exposing employees to electric shock hazards.
